Most insurances cover the cost of your visit (however, prices may vary per insurance. If you have a large deductible, if you have a co-pay or co-insurance, you will be responsible for the charges). If you choose to not charge your insurance, we don't take your insurance, or you want to self-pay, the pricing is noted below:

***Please also note that Self-Pay Pricing is LOWER than what is billed to insurance. 

InBody570 weigh in only: $30/weigh-in (unless a part of weekly weigh-ins as noted below)

First Appointment

Effective August 1st, 2023: New patient appointment $250-This includes a 60-minute appointment and InBody composition analysis. 

If you would like to weigh-in weekly with the InBody composition analysis outside of your normal scheduled visit, it is an additional $25/weigh-in.

**Labs and medications are not included in the monthly fee, but may be covered by your medical insurance.

Second Appointment and After

After your first appointment, we will continue to see you every 2 weeks for the first 3 months, after the first 3 months, there is an option of going to monthly appointments or staying at every two weeks for consistency and accountability.  There is also an option of weekly appointments if desired. 

2nd visit and after: $125/visit-this includes a 25-minute provider visit

If you would like to weigh-in weekly with the InBody composition analysis outside of your normal scheduled visit, it is an additional $25/weigh-in

Maintenance: (If last medical visit <1 year ago)

  • Visits vary based on provider and patient's needs

Total: $125 per visit

Restart Appointment (if last medical visit > 1 year ago but < 3 years ago):

Total: $250 which includes a 50-minute provider appointment with In-Body Composition Analysis.

**After weight goal is met, I encourage you to return to the clinic for a followup visit if you gain 5 or more pounds to get back on track.

You will be paying for each month of care at the agreed rate for appointments made or missed. If you need to cancel an appointment, please call at least 24 hours in advance to reschedule. If you do not, you will forfeit that appointment. If you have an extenuating circumstance, please contact me so that I may accommodate your needs. If you are unable to make it to the clinic for your scheduled appointment, you may ask for a telemedicine visit during your regular appointment time or at another time if it is available.

One of the most important components of success is regular clinic appointments. For this reason, I highly recommend that you make all of your scheduled appointments in advance.

Additional Services:

Sport Physicals: $50 cash pay (if not covered by insurance)

CDL/DOT Physicals: $150 cash pay only

Scout and Missionary Physicals: $150 cash pay (some insurances may cover this as an annual exam. We also offer optional in-house lab (Hgb) that is required for missionary physicals for an additional $25 cash pay). TB testing will still need to be done at the lab.

Forms of Payment

We accept Visa, MasterCard, Amex, Discover, Debit Cards, Venmo, PayPal and Cash. We also accept HRA and HSA card payments.  We accept checks directly from your bank via bill pay or Zelle.  Payment and balances are due at the beginning of your appointment.
